BY BOOKING IT IS TAKEN YOU UNDERSTAND GOVERNMENT GUIDELINES WITH REGARDS TO CORONAVIRUS. PLEASE ONLY BOOK ONLINE IF YOU DON'T HAVE CORONAVIRUS SYMPTOMS.
1.
a) I confirm I understand government guidelines with regard to covid-19 and isolating.
I do not have any signs and symptoms of coronavirus
ie new cough, high fever.
I DO NOT HAVE
• a high temperature – this means you feel hot to touch on your chest or back (you do not need to measure your temperature)
• a new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)
• loss or change to your sense of smell or taste – this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal
IF I DEVELOP ANY OF THESE SYMPTOMS I WILL NOT ATTEND THE CLINIC.
b) The clinic has taken precautions to help prevent any spread of virus and protect the public. Although extreme precautions have been taken in our clinical setting, we cannot guarantee there is no risk. I confirm I am aware of government guidelines regarding coronavirus and am receiving care at my own risk. If I am in a vulnerable group I am receiving care at my own risk even though the clinic has advised against this and advised me to shield. I give consent to have chiropractic care during the current pandemic.
c) The new patient deposit (booking fee) reserves the appointment for you. This fee is non-refundable as once booked, this time is taken out of the diary so no-one else is able to book. Once the deposit is paid, you have to pay the remaining balance at your appointment. i.e. If booking fee is £40 you pay 50-40=£10 at the appointment. Patients who are too late to be seen or miss their appointment will lose the deposit and have to pay it again if they would like to rebook as a new patient. Unless self-isolating.
2. You must ensure you keep and are on time for your appointment. Missed appointments and patients who are too late to be seen will incur a fee and will lose their deposit. If you are unable to attend your appointment, please give us 24 working hours’ notice. You are able to change your appointment online. For Monday appointments please notify us on Friday by 10:00 am. Failure to do so will result in you being charged a full cancellation fee. Unless self-isolating.
3. The description of the services as set out in our website, or any other form of advertising, is for illustrative purposes only and there may be small discrepancies in the service supplied as the chiropractor will decide at each visit which actions are clinically necessary and best for the patient.
4. In case of emergency, or if there is any reason the clinic is unable to keep the appointment the clinic reserves the right to close and reschedule your appointment. I.e illness, emergency or act of god, we will contact you as soon as possible to rearrange to the next available date if necessary.
5. Please ensure you have selected the correct appointment, our online booking system reserves the appointment for you. If you have booked a standard visit for adjustment without having had your initial consultation and examination and report of findings you will not be seen and still be liable to pay for the appointment.
6. You must pay for your appointment in full, when you attend it. In some cases we may ask for payment upfront ie if you have missed an appointment, or if the same appointment is repeatedly rescheduled.
7. Patients who cancel an appointment several times, or patients who reschedule the same appointment/s more than once may be asked to pay for future appointments in advance. Patients who book an appointment several weeks ahead and then cancel short notice will be asked to pay for future appointments in advance or may not be re-booked as per clinic policy. We reserve the right to cancel someones appointments in the diary if someone repeatedly cancels or reschedules an appointment or if we feel the patient is not following our advice or any recommendations provided, or if it is in the patients best interest.
8. The first visit - the new patient appointment, involves a detailed consultation and examination only. On the second visit (after the Chiropractor has had a chance to analyze the findings and formulate a treatment plan) a report of findings will be given followed by treatment (if safe).
Please note: The verbal report of findings given after the initial consultation must be given within 2 months of the initial consultation, otherwise the time period has been too long and you will need to be reexamined.
9. I understand this is a private practice and all outstanding balances must be settled at my appointment. I consent to pay the current
fee for each appointment. If I am too late to be seen or if I fail to show for my appointment or fail to give 24 working hours’
notice of cancellation I understand the clinic reserves the right to charge the current full appointment fee and this fee may be processed
from my card details the clinic has on file
10. Duration varies with appointments.
